If print, slide or digital file does not accurately represent your work, please explain, enclose additional pages as needed. PLEASE NOTE that work not accurately represented may be subject to withdrawal from the show, at the juror’s discretion. Please make copies of this form as needed.

ALL works must be picked up (or prepaid for return shipping) by closing time on these dates without fail. Working with Artists cannot guarantee the safety of any works left in our gallery. No storage service is available and a fee of $20/day will apply for works left in our space past the dates shown here.
